The contact lens formulary is listed on the portal under “reference tools”. If the lens the patient is getting is on the list it is supplied by us, if it isn’t you will supply it using the member’s allowance. Typically, Superior Vision providers supply their own contact lenses.
If you are unsure as to which portal to use, please use the Versant Health option.
